Medical Examination of Third-Country Nationals Subject to Visa Requirements for the Extension of Temporary Stay
A third-country national subject to visa requirements who has been residing in the Republic of Croatia for less than three years must submit proof of a completed medical examination when applying for an extension of temporary stay. The proof of the completed medical examination must not be more than 90 days old.
The medical examination must be completed no more than 90 days before submitting the application for an extension of temporary stay. It is carried out at the Teaching Institute of Public Health of Primorje-Gorski Kotar County in Rijeka or at one of its branch offices, depending on the applicant’s place of stay.
Note: Other visa-required third-country nationals applying for an extension of temporary stay who do not have an employer should contact the appropriate email address for further information, depending on their place of stay.
Important information for employers
The employer arranges the medical examination, completes the referral form and sends it to the email address of the location where the worker will undergo the examination. Once the referral form has been received, the Institute schedules the appointment and issues a quotation.
The employer covers the cost of the medical examination and any required tests. The applicant must bring proof of payment to the examination, together with the other documents listed in the instructions.
Booking the medical examination
Examinations in Rijeka
The completed referral form should be sent to: stranci@zzjzpgz.hr
Examinations are carried out at the Teaching Institute of Public Health of Primorje-Gorski Kotar County, Krešimirova 52a, Rijeka, at the Department of Epidemiology of Infectious and Chronic Non-communicable Diseases.
On arrival, please report to counter 4 or 5, located on the ground floor to the right. The medical examination takes place in Examination Room 5.
Examinations at branch offices
Medical examinations are available at the branch offices in Cres, Opatija, Krk, Delnice, Mali Lošinj and Rab.

Preparing for the medical examination
Before attending the examination, please read the preparation instructions and gather all required documents.
Please bring the following to the examination:
- your passport or another identification document;
- the completed referral form from your employer;
- proof of payment for the examination and required tests;
- your medical records and proof of vaccination, if available;
- a stool sample, if you are from Asia, Africa or South America.
You do not need to fast before the medical examination. If you do not have any of the documents listed above, please still attend the examination and inform the healthcare staff.
What does the medical examination include?
The medical examination includes:
- a discussion of your health and an epidemiological history;
- a review of available medical records and an assessment of vaccination status;
- a physical examination by a doctor;
- blood tests;
- microbiological stool testing for applicants from Asia, Africa and South America;
- a chest X-ray.
Based on the medical and epidemiological history, available medical records and examination findings, the doctor may request additional tests and/or vaccination.
The mandatory examination is carried out to rule out active tuberculosis, carriage of Salmonella Typhi and Salmonella Paratyphi, and infection with HIV, hepatitis B and hepatitis C.
Medical examination process
The medical examination is completed in several steps:
- medical examination and review of medical records
- laboratory tests
- chest X-ray
- additional tests and/or vaccination, if recommended by the doctor
- return to the Institute or branch office to complete the procedure and collect the documentation.
Applicants undergoing the medical examination in Rijeka will be referred for a chest X-ray to the Health Centre of Primorje-Gorski Kotar County, Park Nikole Hosta 4, Rijeka.
Applicants attending a branch office will be informed during the examination where the chest X-ray will be performed.
Medical examination fees
The employer covers the cost of the medical examination and required tests, based on a quotation issued by the Institute. Before attending the examination, the applicant must have proof of payment.
The mandatory medical examination costs:
236.03 EUR medical examination, chest X-ray and serological tests;
251.49 EUR medical examination, chest X-ray, serological tests and microbiological stool testing for applicants from Asia, Africa and South America.
The final cost may be higher than the basic fee if the doctor orders additional tests based on the medical history or examination findings. Additional tests are charged separately according to the Institute’s current price list.
Documentation issued after the examination
After the medical examination, the applicant receives confirmation that they attended the examination. This confirmation serves as proof of attendance until all required tests have been completed and the final documentation has been issued.
Once the medical examination and all required tests have been completed, the applicant will receive:
- a certificate confirming completion of the medical examination,
- a medical certificate.
Where appropriate, the documentation may include a recommendation for further medical evaluation and a catch-up vaccination plan.
The certificate confirming completion of the medical examination must be kept throughout the applicant’s stay in the Republic of Croatia.
